CP2 What to note in the records

Following the appointment it is important that:

  • for firstScan:
    • the sonographer documents the presence of a chaperone.
    • the chaperone’s name is written on the scan report.
  • for Window to the Womb:
    • as this clinic does not conduct examinations involving the genitalia, formal documentation is not as pertinent.
    • it is at the the scan assistant and sonographer’s discretion whether they detail their name on the Fetal Well-being Report.

Why is it important? If an accusation of improper behaviour is made several months or years later and there is no record of who acted as a chaperone, it would be difficult to recall who witnessed the examination.
