firstScan pre scan questionnaire

The pre scan questionnaire is required to be completed before any scan starts. This document gathers important information about the client and her pregnancy to ensure that the sonographer and the clinic team are fully briefed before and after the scan.

• Collects key information of the woman’s history or symptoms for the sonographer.
• Confirms that the woman has seen the Terms and Conditions.
• Confirms that the woman agrees for us to discuss her medical details with an NHS care provider.
• It is a mandatory form and MUST be completed before any scan. If the form isn’t completed – the scan can’t take place!
