Window to the Womb Well-being Report
1. Customers are given this document when they arrive for their appointment and are asked to read the Terms and Conditions and the Technology and Safety information (which will also be given to them on arrival).
2. The customer must sign the document before her scan. She is signing to confirm the following:
  1. That she has seen our Terms and Conditions.
  2. That she has had the opportunity to read the Technology and Safety information.
  3. That they understand what is involved in an ultrasound scan and that they know of no reason why it wouldn’t be suitable for them.
  4. That they give us consent to pass on their medical information to the NHS if we need to do.
